What is included

Published one-off fees for analysing and cryopreserving a single semen sample at German university andrology labs, fertility clinics and sperm banks mostly run 250-515 EUR (Kinderwunschzentrum Dresden 250 EUR incl. 3 months storage, Ludwigshafen 285, Berliner Samenbank 300, Uniklinikum Dresden 342, UKSH Kiel ca. 370, Erlanger Samenbank 410, Cryobank München 430, Ulm ca. 500, UKSH Lübeck ca. 515); the legally required HIV/hepatitis bloods (Anlage 4 TPG-GewV) are usually billed separately at about 70-100 EUR and a consultation can add 70 EUR. Storage is commonly billed per half-year (180-200 EUR) or per year (250-430 EUR); outliers are European Sperm Bank Hamburg (825 EUR bundle including bloods and the first year, then 175 EUR per year or 1,000 EUR for 10 years) and Kinderwunschzentrum Dresden (65 EUR per month after month three, about 780 EUR a year). Multi-year discounts are rare outside European Sperm Bank; Erlanger Samenbank adds 19% VAT for purely elective (non-medical) freezing and storage (487.90 / 499.80 EUR), and several clinics quote GOÄ-based guide prices ('ca.'), so exact invoices vary. Where a bundle includes the first storage year (ESB) or the first three months (KWZ Dresden) this is noted per clinic rather than double counted.

Public funding and insurance

Since the Terminservice- und Versorgungsgesetz of May 2019, § 27a Abs. 4 SGB V gives statutory-insured (GKV) men up to their 50th birthday (women up to their 40th) a right to cryopreservation of sperm or testicular tissue when an illness and its germ-cell-damaging treatment (e.g. orchiectomy, chemotherapy, gonadal radiotherapy) make it medically necessary; the G-BA Kryo-Richtlinie detailing this took effect on 20 February 2021 (amended 18 August 2022) and clinics have billed it as a standard GKV benefit since 1 July 2021. Cover includes preparation, infection screening, extraction, processing, transport, freezing, storage and later thawing, but per Kryo-RL § 3(3) the entitlement ends at the man's 50th birthday or on death, and it is not retroactive for storage begun before July 2021. Elective ('social') freezing, pre-vasectomy freezing and storage without a medical indication are self-paid; private insurers (PKV) decide by tariff, and coverage for trans people before gender-affirming treatment was still legally contested (Bundessozialgericht case B 1 KR 28/23 R) at the time of checking.

Storage limits and consent

German law sets no statutory maximum storage period for a man's own sperm; UKM Münster describes storage as 'nahezu unbegrenzt' (practically unlimited), governed by the storage contract (Verwahrvertrag) with a tissue establishment licensed under §§ 20b/20c AMG and the Transplantationsgesetz, which typically allows disposal when fees go unpaid or contact is lost. Posthumous use is effectively banned: § 4 Abs. 1 Nr. 3 Embryonenschutzgesetz makes knowingly fertilising an egg with a deceased man's sperm punishable by up to three years' imprisonment or a fine (the woman treated is not punished), and GKV-funded storage ends on death (Kryo-RL § 3(3) Nr. 2). Regulator: Länder (state) authorities licence sperm banks and andrology labs as tissue establishments under §§ 20b/20c Arzneimittelgesetz (AMG) and the Transplantationsgesetz; Paul-Ehrlich-Institut (PEI) is the federal authority for tissue preparations; the Gemeinsamer Bundesausschuss (G-BA) sets statutory-insurance cover via the Kryo-Richtlinie.

Frequently asked questions

How much does it cost to freeze sperm in Germany?
Typical published prices are €250–€515 for the initial analysis and freezing of one sample and €250–€430 per year for storage, so ten years (the one-off fee plus nine further years of storage) comes to roughly €2,500–€4,385.
