What is included

A long-established sperm bank such as Fairfax Cryobank charges $375 to process and freeze one unwashed specimen ($485 prewashed), plus a $150 consultation and a $140 infectious-disease blood panel; hospital and fertility-clinic programs commonly quote $500 to $1,000 for analysis and freezing. Storage is the larger cost: Fairfax charges $530 a year per account (prepaid 10 years: $3,300), and Legacy's 2026 cost guide estimates $250 to $700 a year at clinics. Some banks bill per sample or per vial instead of per account, which multiplies the fee if you freeze several samples. Ask whether the first year is included and whether storage is per account, per sample or per vial.

At-home (mail-in) option

Mail-in services available in United States: Legacy, Fellow, CryoChoice, Sppare, ReproTech OverNite Male, Fairfax Priority Male. Expect $439–$700 for the kit, analysis and first year of storage, then $149–$245 per year. Mail-in services bundle the kit, overnight shipping, analysis, freezing and usually the first year of storage: Fellow $199 plus $240 for the first year, Legacy $540 for one sample and one year, ReproTech $575, CryoChoice $595, Sppare $700. From year two storage runs $149 to $245 a year. All ship within the United States only. Compare the kits.

Public funding and insurance

Around 22 states require certain health plans to cover fertility preservation when medically necessary treatment, such as chemotherapy, may cause infertility (RESOLVE state list, 2026). Laws differ on whether storage is covered, and self-funded employer plans regulated under ERISA are usually exempt. Employer fertility benefits (for example Progyny, Carrot or Maven) and cancer programs such as Livestrong Fertility discounts or sperm-bank oncology rates can reduce the cost; elective freezing, for example before a vasectomy, is normally self-paid.

Storage limits and consent

There is no federal maximum storage period: storage is governed by your contract with the bank, which typically allows disposal after written notice if fees go unpaid. Sperm banks register with the FDA as tissue establishments (21 CFR Part 1271); samples stored for your own or an intimate partner's use are labelled accordingly. Use after death depends on state law and your written consent. Regulator: FDA (tissue establishment registration, 21 CFR Part 1271); laboratories certified under CLIA.
