What is included

Published prices for freezing one semen sample at a private Spanish clinic run from about 150 EUR (URE Centro Gutenberg, Malaga; Clinica Fertia, Fuengirola) to about 285 EUR (FIV Valencia), with a prior semen analysis (seminograma) billed separately at 80-110 EUR where it is not bundled, so a first-sample total of roughly 250-365 EUR is typical (Instituto CEFER, Barcelona: 95 EUR seminograma + 270 EUR freezing). Several clinics include the first year of storage in the freezing fee (URE Centro Gutenberg, FIV Valencia, UR Hospital Inmaculada), after which annual custody fees of 200-240 EUR are the norm, rising to 360 EUR at some hospital units. Infectious-disease serology (HIV, hepatitis B/C, syphilis, required under Ley 14/2006) is usually excluded. Public funding and insurance

The Spanish National Health System (SNS) common services portfolio (Real Decreto 1030/2006, Annex III, section 5.3.8.3.d as updated by Orden SSI/2065/2014) covers cryopreservation of gametes for the patient's own deferred use to preserve fertility, but exclusively on medical indication: people at risk of losing reproductive capacity because of gametotoxic treatments (e.g. chemotherapy or radiotherapy) or conditions causing primary testicular failure. Preservation requested only for personal or social reasons is excluded, and the Order does not state how many years of storage are funded; regional health services deliver the service through public hospital sperm banks (for example Fundacio Puigvert in Barcelona, Spain's first public sperm bank since 1986). Some private groups add their own programmes: IVI gives oncology patients aged 18-40 free gamete storage for 5 years, and Dexeus Mujer freezes semen free of charge for oncology patients diagnosed and treated at its Barcelona centre.

Storage limits and consent

Ley 14/2006 sobre tecnicas de reproduccion humana asistida, Article 11.1, allows semen to be cryopreserved in authorised gamete banks 'durante la vida del varon de quien procede', i.e. for the man's lifetime, with no numeric maximum; clinics describe storage as indefinite and renewed under an annual custody contract, so stopping payment is governed by that contract rather than by statute. Article 9.2 permits posthumous use only within 12 months of death and only if the man consented in a public deed, will or advance directive; Article 11.6's two-yearly consent renewal applies to pre-embryos. Regulator: Comisión Nacional de Reproducción Humana Asistida (CNRHA), Ministerio de Sanidad; centres are authorised by the Autonomous Communities and listed in the Registro de centros y servicios de reproducción humana asistida (service code U.29 Banco de semen).
