What is included
Private clinics typically charge CZK 5,000 to 6,000 for freezing a sample with one year of storage (Reprofit Brno CZK 5,900 for all straws plus one year, excluding infectious-disease tests; ISCARE Prague CZK 6,000 including one year; Sanus Hradec Králové CZK 5,000 for self-payers). Gennet Prague charges CZK 8,100 including HIV, hepatitis B and C and syphilis tests, the semen examination, freezing and one year of storage. Storage after the first year costs CZK 2,000 to 3,500 a year, and more if screening is positive or missing. Several clinics publish prices in euros for international patients (Reprofit Ostrava: €305 including one year, then €205 a year).
Public funding and insurance
Public health insurance generally covers sperm collection and freezing for a medical indication, such as cancer treatment; storage may be charged after an initial period (for example, VFN Prague charges a storage fee for each started year). Freezing and storage for personal reasons are self-paid.
Storage limits and consent
Act No. 373/2011 Coll. on specific health services sets no explicit maximum storage period for a man's own sperm; storage runs under your contract with the tissue establishment. The law requires written consent before each use in treatment, and it does not provide for use after death. Regulator: State Institute for Drug Control (SÚKL) licenses tissue establishments; Ministry of Health.
